Medication Authorization Forms

Dear District Families,

Welcome back to the 2022-23 school year! I wanted to bring to your attention to a couple of new forms that are going to be required for any student receiving medication at school. The first form is prescription medication authorization, which requires a health care provider signature. This form is for students that are prescribed a medication from a health care provider and may have to utilize the medication during school hours. Examples include, epi pens, inhaled medications, antibiotics, antiseizure meds to name a few.

The second form is non-prescription (over-the-counter) medication authorization. This allows students to receive the following medications: Tylenol, ibuprofen, Benadryl and tums during school hours if deemed necessary.

Medication forms are available on the district’s website and Facebook page, and in the Outpatient Department at BBAHC.

In addition to the medication forms, there is also an Asthma Care Plan for those students who have a diagnosis of Asthma.
