Welcome to Dillingham Middle High School. the following forms enclosed in the Enrollment Packet need to be completed in order to enroll your student in our School. This packet includes the following forms: Records Request, Contact and Medical Permission, Federal Programs, Free and Reduced Lunch Application, Internet Use Agreement, Laptop Checkout, Bilingual Questionnaire, and Locker Agreement. In addition to these forms please bring with you a copy of your child’s up to date Immunization Records signed by a doctor or nurse and a copy of his/her birth certificate.

One to One Student Laptop Program
Dillingham Middle High Students are each given a laptop for in school and take home use to assist in their learning. The form in this down load section is an overview of the program and the expectations on the students handling of their laptops.
This is one of two forms needed for students a student to aquire a laptop. The other form needed is the Student Internet Agreement. Read and sign both of these forms and return them to the Technology Office with a student Laptop Usage Fee. (This form is also included in the Enrollment Packet)

In the unlikely event that your laptop is damaged. Download this form, fill in as much detail as possible, and return the form and the laptop to the Technology Department as soon as possible.
